Address Update Form (AUF)

It is important you keep in contact with our office; we encourage you to keep your mailing address current with our office at all times to assure you receive important information from the community. Upon returning:

Complete all areas of the form
The form must be notarized when returning by mail or fax
Please include all minor children who receive mail at this address. If you have guardianship over an enrolled member(s), please provide official documentation, i.e. Court Order or Power of Attorney for our records and include them on your AUF.

Tribal ID Request Form

Tribal Identification cards are used to verify your membership with the Gila River Indian Community. This form is used when requesting a tribal ID through mail, upon returning:

Complete all areas of the form, including Tribal ID Instruction sheet with signature
The form must be notarized
Include a head shot photo, example:

Photo must have solid background (no scenery)
No caps, sunglasses or bandanas
Include the signed Tribal ID Instruction sheet, using a black ink pen (preferably fine felt tip or fine marker style pen)
Include an $8.00 money order payable to Gila River Indian Community

Authorization to Release (ATR)

Authorization to Release form is used for the purpose of requesting the following:

Certificate of Indian Blood
BIA 4432 Employment Form
Game & Fish Wildlife Form (Eagle Feather permit)
Complete all areas of the form
The form must be notarized when returning by mail or fax.

Documents Required for Membership with the Gila River Indian Community
Enrollment - Office Information
It is the Parent/Guardian(s) responsibility to provide the Enrollment Department with the following documents to enroll a child with the Gila River Indian Community, in accordance with the Enrollment Ordinance GR-02-05 Section 8.106 Enrollment (A):

1. Must have an Original Certified Birth Certificate;

2. Must have an Original Social Security Card;

3. If one of the parents of the applicant is currently a member of another federally recognized Indian tribe:

a. Documentation verifying non-membership in another federally recognized tribe; and

b. Certification of Indian Blood degree

4. Photo Identification Card required for parent(s)

According to our Standard Operation Procedure,

"THE PARENT WHO IS THE ENROLLED MEMBER OF THE GILA RIVER INDIAN COMMUNITY MUST BE THE ONE TO SIGN THE ENROLLMENT APPLICATION. IF THIS IS NOT POSSIBLE, COURT DOCUMENTS MUST BE SUBMITTED TO VERIFY CUSTODIAL RIGHTS FOR THE PERSON SUBMITTING ON BEHALF OF THE APPLICANT. IF NO CUSTODY IS DETERMINED, AND THE MOTHER OR FATHER IS A NON-MEMBER OR NON-INDIAN THE PARENT THAT IS ENROLLED WITH GILA RIVER INDIAN COMMUNITY WILL BE NOTIFIED THAT AN ENROLLMENT APPLICATION HAS BEEN SUBMITTED ON BEHALF OF THE APPLICANT. THE SAME WILL APPLY TO RELINQUISHMENTS, PETITIONS, DIS-ENROLLMENTS, DENIALS, AND REMOVAL."
Therefore, you will need to obtain the enrolled parent's signature or submit the court order regarding custody.

Membership Requirements

When applying for membership with Gila River Indian Community (GRIC) the following is required in accordance with Title 3 of the Tribal Code:

  • A biological parent MUST be currently enrolled with GRIC
  • Must be at least 1/4 Native Blood
  • Submit an Original Birth Certificate & Original Social Security Card with application
  • If a parent is enrolled with another Federally Recognized Tribe, Verification of Non-Membership for the applicant as well as a Certified Degree of Indian Blood for the Parent who is enrolled with the Other Federally Recognized Tribe.

The Enrollment Office Accepts Applications on a daily basis and are only accepted in person (no mail-ins), and we process the applications Bi-annually. Applications submitted January-June 30th are processed in July and August; applications submitted July thru December 31 are processed in January and February.
